NOTE: In order to receive funding related to your FEMA Public Assistance Grant, you must first designate an Authorized Agent. To do so, you need to appoint an individual, either by Letter of Appointment or Resolution. This individual will then complete and sign the State/Local Agreement and W-9. All forms are available below. Originals should be mailed to Attn: Laura Adcock, State PA Officer, Ohio EMA, 2855 W. Dublin Granville Road, Columbus, Ohio 43235.
